Locations:

  • Aylesbury (Quarrendon) – 4 posts
  • High Wycombe (Castlefield) – 4 posts
  • Buckinghamshire-wide (Street Homelessness focus) – 1 post

Hours: Flexible, part-time (including some evenings and weekends)
Contract: Fixed term for 12 months (with the possibility of extension)
Salary:  In the range of £31,000- £34,000 depending on experience

We’re recruiting 9 x Community Health and Wellbeing Workers (CHWWs) to join our Integrated Neighbourhood Teams (INTs) in Buckinghamshire.

  • 4 x CHWWs will be based in Aylesbury (Quarrendon)
  • 4 x CHWWs will be based in High Wycombe (Castlefield)
  • 1 x CHWW will work across the 2 areas and have a focus, supporting people experiencing street homelessness and working closely with services and organisations supporting this community.

The Homeless CHWW will need to work alongside partners already offering support and services to the homeless population, this will include joint outreach and co-working to complement the existing offer and minimise duplication of efforts”

These new and exciting roles act as a bridge between health, social care, and community services — helping people navigate support, access the help they need, and live healthier, happier lives.

You’ll work directly with individuals and families, listening to their stories, understanding their needs, and helping them find practical ways to improve their wellbeing. Whether it’s connecting someone to local activities, supporting them to attend medical appointments, or signposting to housing, financial, or mental health support — your work will make a real, lasting difference.

This is a unique opportunity to shape a new model of community health and be part of a growing national movement of CHWWs tackling health inequalities at neighbourhood level.

What You’ll Do

  • Build trusted relationships with households or individuals in your local area through regular outreach visits.
  • Listen, support, and identify health and social needs — helping people find and access the right services.
  • Manage a caseload, following through on agreed actions to support individuals and families to reach their goals.
  • Work collaboratively with GPs, social prescribers, local authorities, and voluntary sector organisations to deliver joined-up, person-centred care.
  • Support people to take part in community-based activities that promote health and wellbeing.
  • Promote key health messages, such as vaccination, screening, and healthy lifestyle choices.
  • Record and share insights securely to help improve services and identify unmet needs.
  • Play an active role in community engagement, helping to plan and deliver events, outreach sessions, and wellbeing activities.

About You

You don’t need previous experience in health or social care — what matters most is your passion for people, your connection to your community, and your desire to make a difference.

We’d love to hear from you if you:

  • Are a good listener, empathetic, and confident talking to people from different backgrounds and cultures.
  • Are caring, approachable, and motivated to help others.
  • Can work independently and manage your own time effectively.
  • Feel comfortable using a tablet or smartphone to record information.
  • Are reliable, positive, and committed to equality, inclusion, and diversity.

Experience in community outreach, advocacy, voluntary work, or health coaching would be an advantage — but full training and support will be provided.

Training and Support

You’ll receive a comprehensive induction and continuous professional development, including:

  • Core training in communication, community engagement, and safeguarding.
  • Skills in health coaching and motivational interviewing.
  • Regular reflective learning sessions and peer support with other CHWWs.
  • Opportunities to shadow services and learn about local initiatives.
  • Access to a work phone, tablet, and supervision from an Integration and Outreach Manager.

EXPECTATIONS: 

  • Knowledge of community and local areas of Aylesbury and High Wycombe.
  • Interviews will be held on the 18th and 19th
  • Candidates must be able to start by January 2026
  • Candidates must be available to undertake Health Coaching training on 15th, 16th 29th and 30th
  • This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
