Community Health and Wellbeing Workers (CHWW) are a new service for residents being introduced in Spring 2026 to selected areas of Castlefield (High Wycombe) and Quarrendon (Aylesbury).

The CHWW teams have been recruited from the local community and will be actively contacting all residents who live in their area to introduce themselves and offer support, helping residents to access health and wellbeing services and sharing healthy living advice.

Every CHWW covers up to 150 homes, and they will be aiming to visit or contact each home every month. During the visits, they will be listening to residents and discussing any issues relating to physical, mental and social health.

The CHWW model of care is:

Comprehensive – CHWWs look after the whole household and will aim to help residents with anything of concern.

Hyper local – CHWWs are recruited from the local area and work with only 120-150 households.

Universal – Everyone in the area is seen or contacted regularly by their CHWW and is offered help where needed.

Integrated – CHWWs work closely with local GPs, local authorities and other community services.
